It is always prudent to submit cancellations or changes on your insurance policy in writing to you provider. If you fax it, keep a copy of the confirmation date stamp and if you e-mail it, retain the record until the action you have requested has been confirmed in writing.

Before you cancel or change your policy requirements, contact your provider and ask for their coordinates or contact information for cancellation of a policy. Get their name, address, zip, e-mail, fax and any other details.
